What are the documents required for Prepaid SIM card registration?

The Service Providers and their representatives would require verification against the original identification of the following: -
 

Malaysian	MyKad issued by National Registration Department
Malaysian Permanent Residents	MyPR card issued by National Registration Department
Malaysian Armed Forces	MyTentera card issued by National Registration Department
Foreigners	Passport, Work Permit Identification cards & Student Identification cards
Other documents issued by the National Registration Department (“NRD”) and Immigration Department of Malaysia (“IDM”)	Temporary Identity Card (MyKas); IMM 13 (limited to the state of Sabah and Wilayah Persekutuan Labuan only and verification is required from IDM prior to activation of services by Service Provider).
