Applicant shall submit a complete AA application form for Amateur Station to MCMC with the following documents:
i.A copy of applicant’s identification card or passport;
ii.A copy of Radio Amateur Examination (RAE) result for Class B license application/ a copy of Morse Code result for Class A license application. (Printed copy of RAE result from MCMC’s website is also acceptable);
iii.Letter of reference by two (2) members of MARTS or Class A amateur radio holders (Appendix A of the form);
iv.Statutory declaration form signed by Justice of the Peace/ Magistrate/ a Commissioner of Oaths (Appendix B of the form);
v.List of three (3) preferable call signs (Appendix C of the form). MCMC reserves the right to assign any call sign in the event the preferred call sign is not available.
vi.A copy of existing AA certificate/ a copy of RAE or Morse Code result (for re-application).
